Maybe the tension is that there is enough societal memory of what it means to care for a child properly in Russia. In contrast to 3rd world.

The low birth rate can be attributed back to Reagan believe it or not. Reagan forced the soviets to consider either guns or butter. They went for the guns. So soviet social services went to crap. That shifted the burden of post natal child rearing back to families. But the russian people were already being stretched too thin so children became a serious burden.

By the time the soviets figured they might be able to pull the guns through Reagan did his brilliant pebbles schtick. That pretty much put the soviets to the point they had to reconsider. By the time they put the breaks on the guns it was too late. Yeltsin came along. Then a year later they had the banking collapse. All those stresses, made child rearing an irrational decision for most.

So you have a declining birth rate for almost 3 decades. Couple that with a health sciences that remained flat on systemic improvements and you get these results. The real key is that a society must have a 2.1 birth rate to remain constant. Anything below that and you are in a unrecoverable downward spiral. The russians have been at 1.9-1.3 for 30 years.

Putin instituted inducements 3 years ago. Mothers receive payments from the state beyond the first child. A paid leave of absence is also provided. Probably won’t be enough.
